Monica Fisk, a Darzi Fellow (Diabetes theme) working for the South London Health Innovation Network at Guy's and St Thomas' NHS Foundation Trust, is currently scoping a project around the safe discharge from hospital back to the community of patients who are on insulin and wants to hear patients' views of how they feel things could be improved.
Have you had an experience where your discharge on insulin could have been better, for example, where you have been discharged on the wrong insulin or wrong dose or are new to insulin and don’t feel you have been given sufficient advice?
If so please contact Monica via monica.fisk@nhs.net
